On October 11, 2004, a serious electrical arc flash accident occurred at SLAC. The accelerators were shut-down and a series of enhanced safety-awareness processes were begun. A US DOE Type A Accident Investigation Board was at SLAC from October 18, 2005 until November 6, 2005. During that process, the Director’s “Safety Comes First” initiative was put in place. All SLAC employees had to complete a review of key safety documents/requirements prior to their return to normal work. SLAC completed a Corrective Action Plan for the DOE report’s Judgments of Need in January 2005. The SSRL accelerator was reviewed for restart of operations by a Validation Team. The Director approved SSRL restart subject to addressing the Validation Team’s pre-restart recommendations. The B-Factory accelerator facility was reviewed for restart of operations by a larger Validation Team. The Director approved the B-Factory restart subject to addressing the Validation Team’s pre-restart recommendations.
